When examining your complete organization (the entire company/governmental organization, or a complete IT, HR, finance, or legal department), how would you answer the following questions?

1: Is your organization under significant pressure to make drastic improvements in terms of costs, flexibility, and/or alignment with the business?
2: When considering projects executed over the last three years, have more than 25% of your initiated projects suffered from one of the following: project cancelled, substantial scope reduction, more than 25% budget overrun, deadline missed by months?
3: Is your organization suffering from high bureaucracy?
4: Is your organization’s environment too complex to understand?
5: Are your employees in a position to comply with the strategies and standards? In other words:
  • Are your employees able to find the instructions amidst the information overload?
  • Are the instructions reasonably up-to-date?
  • Are the instructions concise and complete?
  • Are important gaps in the instructions closed within a reasonable period?
  • Are cultural obstacles overcome?
  • For each instruction: Have most of the possible situations been foreseen and tested (including direct and indirect implications)?
  • If the answer to the previous question is "no": Is there sufficient freedom and guidance for employees to make decisions in unforeseen/untested situations?
  • Are the instructions executable (i.e., have they been verified by subject-matter experts representing those expected to be compliant)?
6: When a solution approach failed: Has your organization used that approach three or more times (for example, the same best practice, method, or technology)?
7: Is your organization using best practices/methods/models without receiving their expected value?

Examples: quality management, project management, processes, "command and control", governance, ITIL. (More than 50 such "models" are in use in IT environments.)
8: Is your organization implementing or planning to implement best practices/methods/models, such as those indicated in question 7, to increase efficiency or to reduce risks in a service environment?

(This can apply to an internal service environment, such as IT, or to an external environment, such as hospitals.)
